Please use this form as a guide to complete your diagnostic assessment. Students should submit one assessment with clearly identified sections as noted below. All assessments must be typed and submitted to the instructor by the deadline.

Diagnostic Evaluation
1. Presenting Problem – Clearly identify the CENTRAL problem(s) that is most obvious from the vignette. Do NOT copy and paste, DO NOT LIST SYMPTOMS. You are to only identify the problems that bring the client in for treatment.
2. Client Information – List the client’s information in a report format. This should include (if listed) Name, DOB, Age, Education, Employment, Marital Status, etc.
3. Diagnosis – With the information provided, please provide a logical diagnosis for the client. DO NOT ADD INFORMATION THAT IS NOT CLEARLY STATED IN THE VIGNETTE. If you make any assumptions, please be able to defend those assumptions. Please use an appropriate DSM-5 diagnosis
4. Supporting Symptoms – Identify what symptoms are present (according to the DSM/textbook) that aided in the selection of the above diagnosis (presenting problem). This information should come directly from the vignette, other than the obvious, DO NOT ADD ANY ADDITIONAL INFORMATION OR MAKE ANY ASSUMPTIONS. LIST ALL SUPPORTING SYMPTOMS!
5. Rule Out Diagnosis – Identify one other diagnosis that you initially “ruled out” but were able to find another diagnosis that best fit the client. Please provide an explanation of why this diagnosis was ruled out. Your textbook defines what “rule out” diagnoses are.
6. Questions – If you could personally meet the client, what questions would you ask to provide a more comprehensive diagnosis? What additional information is missing that you feel is needed? You should ask 3-5 comprehensive questions.
7. Treatment – With the disorder you diagnosed, what are possible treatment options? What are effective techniques you would offer the client? (Remember!! Clinicians DO NOT GIVE ADVICE). You do not have to list medications, as you are not a psychiatrist. However, you can list a referral to a psychiatrist as a part of your treatment plan.

iii. Determine if referral is needed for medication/hospitalization
