Schizophrenia spectrum and other psychotic disorder

HPI: Client A is a 50 y/o Caucasian male with diagnosis and history of schizophrenia. He was accompanied by his daughter to the clinic for complaint of demons whispering in his ears, not sleeping, and not coming out of his room for four days now. Mr. A stopped taking his medication when he lost his job six months ago (he worked in a restaurant) and ran out of medication three months ago. Patient was reported to have started drinking 2 months ago. Patient has been in and out of jobs. He has been working at the restaurant for five years before he was laid off. He lives with his aged mother and 20 y/old daughter. Patient was hospitalized about 10 years ago for 20 months. During the interview, the patient reported that he had a device implanted in his brain by the government. He wants the device removed and the demon is telling him where to get it removed from. Patient is a veteran and feels the government is trying to send him on a sensitive and classified mission. Patient has been observed to respond to internal stimuli lately, talking to invisible people. He reported the voices are telling him to go meet with the president at the white house. At other times, the voice is telling him to stop taking his medications because it contains poison. Patient stays up most nights and insists he does not want to go for the classified mission. Patient was on Ziprasidone. Family would want to restart patient on ziprasidone because it was well tolerated by patient.
