Complete the 7 Day Physical Activity Log (Appendix 1), defining activity, intensity and length of duration. Remember to include structured exercise/activity sessions but also incidental activity.
Pre Exercise Screening Questionnaire (Links in Appendix 2 and the assessment area), Complete both Stage 1 and Stage 2 to the best of your ability without requiring medical professional support. Then utilise the user guide to identify risk level. Answer the additional questions under the heading, Self-Reflection Questions. Use the word count suggestions to guide how much detail is required.
