My clinical exemplar is about one situation which happened last year, while I was working at Monmouth infusion. It shows to I navigated complicated issue while manage to increase pt’s satisfaction. It was Wednesday, almost end of the day and I picked up Mrs Abby as my next pt for her for her 1st time intravesical chemo treatment. Usually intravesical treatments are scheduled on Tuesday and Thursday morning when we have presence of our Urology team that can address any issues that could arise (difficult accessing pt, high post void volume ect), I raised my concern with charge nurse Lisa because I knew that scheduling pt this late and off usual days could cause difficulty if we have come to an issue that provider had to be contacted. Usually is such a specialized situations MOD differing issue to be addressed by ordering Dr. I brought Mrs Abby to treatment room, completed my assessment and try to investigate the situation that Mrs Abby was in. I find out that she was retired Nurse with 35years of experience in local hospital and that this was her 3rd recurrence of her bladder cancer and 3rd time around starting chemo. Also, thar she was main caretaker of her husband who has advance stage of Dementia and that she was unable to drive long distance and only time she could make appt was after 5pm during weekday that her daughter could drive her after her work. At the beginning pt expressed quite dissatisfaction for been waiting almost an hr pass her appt. I apologies and explained that unfortunately sometimes all our treatment chairs are occupied which causes delay and extra waiting time. After completing assessment, education and independent double check I proceeded to perform catheterization to instill chemo through foley cath. Pt had past Hx of bilateral hip replacement and recent hx (last week) of hip dislocation. Due to pt history I was unable to use stirrup to avoid excessive abduction of hips, due to a somewhat difficult anatomical location and unable to use stirrup, It was somewhat difficult it insert foley cath, therefore I had to call my coworker to hold and increase visibility for me to successfully perform procedure, pt had over 150ml urine output which caused additional delay because due to out policy if post void output is over 150ml we have to inform ordering MD and proceed accordingly what provider suggests. I escalated issue again to charge nurse to inform provider while remaining with pt whole time, providing accurate real time update to situation as well as emotional support. Fortunately, md responded on timely manner and I was able to complete instilling chemo per md’s order. I remained with pt next 2 hrs post procedure while pt waited for dwelling time and I was able to calm pt down. After pt left I had dictation with charge nurse what could be done to avoid similar situation in future. I was part of multidisciplinary email chain that included, provider, management, admen, treatment team to come up with a plan to avoid farther complications, since pt wasn’t able to come any other days or go to NYC for her treatment, ordering dr provided own cell # to get in contact with him in case of future issues arise, pt had biweekly treatment for next 6 weeks and I always volunteered to be her nurse or has my coworker with me to show my technical approach to access to make pt’s experience easer during her visits when I wasn’t there. Out of 12 tx, I had pt 8 times and during this time I developed close relationship with her. Who would think after out 1st encounter that we would be taking about her upcoming vacation after chemo, or challenges she faced daily based to care for her husband or stories about her enormous experience she had as a been a nurse for 35years. I am very honored and pleased that end of last treatment Abby filled Caring Heart paper to thank me for my professionalism. I am very happy to be part of Abby’s treatment team and even though 1st day of treatment she was very dissatisfied, saying that it was the worst experience she had due as a pt in her life, I am very proud to be a part of a treatment team who was able to make adjustments, address pt’s unique situation and provide pleasant experience for remaining of the treatment period. In this particular situation because I escalated issue to a charge and administration level and also was able to advocate for pt as well as to share my experience with my team to make pts future treatment encounters free from complications, it made difference and pt had pleasant and smooth treatment experiences for remaining of her treatment. I am proud to be a part of a increasable team and an organization which evaluates individual pt’s needs and goes above and beyond to provide, safe, high quality care while assuring pt’s satisfaction.
