Caring for the Nguyens
Recall that you have written the following nursing diagnosis for Mr. Nguyen: Overweight related to inappropriate food choices and serving size, as evidenced by body mass index (BMI) of 28.5
Based on the outcomes you wrote in Chapter 5, Planning Outcomes, identify four possible interventions to address the diagnosis and outcomes. Review the interventions, and identify two that would be most appropriate for Mr. Nguyen given all of the information you have about his health status. Write a nursing order for each of the two interventions you identified above.

ANSWER
Caring for the Nguyen
Nursing interventions are those actions, measures, strategies taken based on clinical judgement and knowledge, such as through the diagnosis and planning outcomes created in Mr. Nguyen’s case that are necessary to achieve the identified outcomes. The outcomes identified included poor health for Mr. Nguyen due to his eating habits, food size, and food choices (Dailey, 2016). The essential goals include effective weight management, which will be achieved through short and long-term goals. In that case, the interventions plausible in adopting based on the outcomes include direct care, in-direct care, interdependent, and independent interventions. In direct care intervention, I will work and interact with Mr. Nguyen directly by providing physical, emotional assistance and patient learning or teaching during his weight loss and management journey. This will occur mostly during the identified strategies such as physical activities and therapies (Treas & Wilkinson, 2013). In the interdependent intervention, the collaboration with other healthcare team members, most notably the therapists will be required. Among the identified strategies of weight management, include dietary, pharmacotherapy, and behavioral therapies, which would require an SME in the fields to provide maximum help.
Short/Long-term Goals/Outcomes | Strategies |
Reduced food size, habits, and choices | Dietary therapy |
Healthy weight loss and improved overall health | Weight loss therapy
Pharmacotherapy Physical activity |
Weight loss management | Combination of all |
In the indirect care intervention, Mr. Nguyen will require the support and care from his family members through the adaptation and implementation of new dietary plans that will help support during his weight management journey. Moreover, in cases, the nurse is absent during physical activity; other team members in the healthcare team can be consulted (Treas & Wilkinson, 2013). Finally, the independent intervention, most notably, the health promotion nursing intervention, will be integrated that will grant increased autonomy with the patient to initiate actions and measures effectiveness. This will include extensive patient teaching and education on the need to take up the treatment plan on weight loss (Fruh, 2017). In cases, Mr. Nguyen fails to take therapies or physical activities; the intervention will involve education on the importance, thus health promotion.
The two most appropriate interventions identified for Mr. Nguyen include health promotion (independent intervention) and interdependent interventions. These two interventions will help maximize the health outcomes, and defined goals as the autonomy of the decisions will help monitor Mr. Nguyen’s health throughout. The help of the SMEs in the therapies is an added advantage for maximized quality of healthcare and wellbeing. These two interventions will help to develop more standardized-interventions and individualized-nursing orders. The table below outlines the standardized-integrations and individualized-nursing order for Mr. Nguyen.
Standardized interventions | Individualized nursing order |
Assess weight and BP | Assess weight and BP biweekly during the scheduled clinic days (Monday and Friday) |

Encourage the attendance of all physical activity and therapy sessions Assist Mr. Nguyen on all-day sessions of his physical activity afterwards between 9:00-10:00. |
