I agree with the assessment conducted in the post explaining the possibilities of having other underlying conditions in the adolescent boy, and the explanation makes physiological sense. Primarily, it is true that the patient’s hypertensive reaction to the antibiotic, amoxicillin given was due to the disruption of physiological processes. As expected, prescribing amoxicillin to the patient should have shortened the length of his strep throat infection, but the reaction experienced could have been caused by bacterial resistance to the drugs (Chaplin, 2010). In the process of resistance as mentioned, cellular destructions are experienced hence resulting in bronchoconstriction. The patient underwent the adverse effects of amoxicillin that resulted in type I hypersensitivity reactions. In this type I hypersensitivity reactions, the IgE antibodies are mediators of the response that result in urticarial effects that range from rashes to severe systematic symptoms such as anaphylaxis (Akhavan, Khanna, & Vijhani., 2020). This explanation is well illustrated and described in your post; hence, I agree with your assessment even more.

However, I had a question on the IgE and IgG antibodies, as explained in your assessment. In your post, you indicated that the patient experienced swelling of tongue, lips, and had breathing difficulties due to IgG antibody reactions that caused inflammations. On the other hand, in my research, the findings lay more on IgE as the most causal factor of allergic overreaction. The patient likely had an allergic reaction to amoxicillin-penicillin, in which the immune system overreacted by producing these IgE and upon travelling to the cells; chemicals were released thus causing the allergic reaction of swelling in the throat (Chaplin, 2010). Nevertheless, I agree with you about the possibility of tonsillopharyngitis in the patient, which caused a severe allergic reaction.
