Purpose of the Study:  The objective of the research was to describe and identify the contemporary nursing practices in the fluid balance recording and monitoring during intensive care as well as the measurement of accuracy (Diacon and Bell, 2014). The study focused on fluid balance management, which is significant in managing intensive care patients. Vital management of critically ill patient demands for a precise evaluation of their fluid balance (Asfour, 2016). Considerably, the evaluation entails proper monitoring of fluid consumption, fluid output, as well as the correct recording and calculation of data.

Research & Design:  The study used a quantitative method that utilized exploratory and descriptive design. The quantitative approach emphasizes objective measurement and statistical analysis of data collected through surveys that describe the phenomenon across a group of participants. The quantitative data was associated with existing clinical practices and how fluid balance components were recorded and monitored. They were collected using the audit tool that was essential in assessing relevant fluid balance records.  The descriptive design aims at systematically and accurately describing the phenomenon to investigate the variable. At the same time, exploratory research entails investigating the phenomenon to offer a better understanding and facilitate additional research during the experiment.

The study was carried out in Intensive care units in three hospitals that belonged to a private hospital group (Diacon and Bell, 2014). The Intensive care units within the hospitals were comparable with regards to admission details for the patients, the policies being applied, and the nursing documentation. The audit tool was used to determine the various facets of the fluid balance sampled. The research method and design were used in establishing the current nurse practices as well as accuracy in fluid balance recording and monitoring. The exploratory and descriptive design has described the nurses’ knowledge and perspectives in intensive care units concerning fluid balance recording and monitoring.

Sample: The study population entailed the critical care patients and the registered nurses that worked in intensive care units that were used in the survey. The sample was acute care patient information. Considerably, the study sample was obtained from the fluid balance data that were predetermined by the following inclusion conditions: the nursing information for the patient’s admission to the intensive care unit for the first 48 hours of stay and patients with ages above 18 years (Diacon and Bell, 2014). The research utilized a simple random sampling technique to select the patient’s records for the audit. The research relied on a sample size of 103 ICU records to evaluate the existing nurse practices and the precision of the fluid balance in checking and recording (Diacon and Bell, 2014). The 103 ICU records were obtained from the critical care units of the three hospitals. The selected samples were divided and selected specifically among the varying units within the three hospitals with the guidance of statisticians.

Data Collection: The data was collected through an audit tool developed from clinical experience and literature. A statistician assisted in the development of the audit tool and ensured that the tool was appropriate and adequate for the collection of data as well as analysis. The audit tool enabled the collection of data concerning the execution of fluid intake and output, recording and monitoring of fluid balance data in intensive care observation data and the variations in fluid intake, the output as well as cumulative fluid balance. The data was documented in the audit tool within the three hospitals.

Data Analysis: Data analysis reveals the results that are deduced based on the collected data. For this quantitative research, data analysis was conducted through statistical procedures to examine the data collected. The original data collection was the 24-h fluid balance total, which was paralleled to the audited fluid balance sum recorded (Diacon and Bell, 2014). The variance in the fluid balance totals was used to indicate the deviation in fluid balance calculation.

Limitations:  Relying on data from intensive care units of only one hospital group limited the study’s generalization. Considerably, having different hospital groups or hospitals from different hospital groups could offer different aspects of fluid balance recording and monitoring. Furthermore, the study used the smile size of the sample that might have influenced the results of the study. Therefore, in-depth research on the inaccuracy of the fluid balance calculation, as well as the investigation of what constitutes the inaccuracy, needs a mixed approach to get more profound perspectives of the nurses and fluid balance calculation.

Findings/Discussion:  The findings of the study have shown a deviation between the controlled calculation and the 24-h recorded fluid balance. In the samples’ audit, 71 recorded fluid balance totals indicate a deviation of 500ml, 14 indicated variances between 100 ml and 500 ml, 7 recorded between 1000ml and 2000 deviation, while 6 recorded a deviation of more than 2000ml (Diacon and Bell, 2014). This indicates that decisions for treatment in the three hospitals’ intensive care units are based on inaccurate data, which may have great negative implications on the health of the patient. The findings of this research indicate a risk for the patients within the hospitals due to the variance in the calculation of fluid balance. Considering the previous research on the effectiveness of fluid balance and its effect on health and patient mortality, the nurses are posing a greater risk to patients. The incorrect fluid balance calculation indicates that the patient management decision is not treating fluid balance as a priority, which leads to negative impacts on the patients (Scales and Pilsworth, 2008). The requirement to offer correct and accurate fluid balance recording and monitoring is vital, thus, standard nursing practices within the Intensive care units should be established to facilitate an accurate and careful approach to fluid balance in managing patients in intensive care units.

Reading Research Literature: Reading the research literature offered more knowledge on the significance of fluid balance as well as the relevance of accurate recording and checking. The current clinical nurse practices have failed to prioritize fluid balance recording and monitoring, with most studies indicating that fluid balances are always incorrect. The fluid balance plays an important part in the management of patients in intensive care units (Scales and Pilsworth, 2008). The precise evaluation of the fluid balance data gathered during the assessment, recordkeeping, and monitoring activities plays an essential role in guiding the nursing and medical interventions intended to reach patient health. The effective control of patient’s health demands for precise assessment of fluid balance status. Inaccurate recording results in negative consequences in clinical management (Jeyapala, Gerth, Patel, and Syed, 2015).  For that reason, critical care nurses need to conduct effective fluid balance recording, monitoring, and recording to offer quality and safe patient care.
