The problem list of the patient

Mr. Nam Nguyen was suffering from multiple chronic illnesses. The diagnosis proved that he was suffering from multiple diseases, which could be attributed to his lifestyle and family history. The diseases he is suffering from are significant causes of death and disability across the globe, and he would need to undergo comprehensive clinical care as a way of dealing with the illnesses. According to the WHO, the risk factors of such diseases include high blood pressure, obesity, alcohol, and tobacco, where they increase the rate of one getting these diseases (World Health Organization, 2020).   Scientific evidence shows that a significant change in dietary habits increased physical activity, and tobacco control can help produce rapid changes in the population risk factors for these chronic diseases (Health- Conditions, 2020). This means that there is a need for Mr. Nam to undergo various lifestyle changes and adopt a healthy lifestyle as a way of staying safe and ensuring that he can avoid getting infected from such.

How is it similar to or different from a problem list that you might generate?

The patient suffers similar symptoms to other persons with chronic illnesses. Most of these symptoms are related to his lifestyle, where it is apparent that he uses substances such as tobacco, which are harmful to his health.

One actual diagnosis is musculoskeletal pain.

One potential diagnosis of Hypertension Obesity.

One wellness diagnosis is prostate cancer.

The NANDA-I labels

Some of the NANDA-I labels include obesity, which is a nutrition attributed diagnosis. The patient has not been living a healthy routine, a factor that triggered the disease. Another label is a risk for activity intolerance, which is due to his failure to undertake physical activities, which help maintain a healthy lifestyle (Treas & Wilkinson, 2013). Other labels identified from the patient include the risk for unstable blood glucose level, Neonatal hyperbilirubinemia, and the risk for neonatal hyperbilirubinemia (World Health Organization, 2020). This is due to his poor routine choices, more so regarding eating and smoking habits.

A statement describing Mr. Nguyen’s nutritional status

The patient’s current nutrition status is wanting and could be one of the triggers to his illness. First, he lacks a well-balanced diet, which has hence resulted in these chronic diseases. His diet consists of junk foods and too many fats, which is a factor that led to his obesity cases. Tobacco is also a factor enhancing his case of hypertension and musculoskeletal pain and could also enhance his chances of having cardiovascular disease, which is in his family history (Treas & Wilkinson, 2013).

Nutrition statement as a three-part statement

The nurse has identified Acute Pain (knees) for Mr. Nguyen. This means that it is likely that he has another medical condition, which may possibly be osteoarthritis. One of the likely causes of the disease is obesity, which is a lifestyle disease and can also be attributed to his family history (Treas & Wilkinson, 2013). The patient’s lifestyle has been reduced, where he evidently consumes junk and fatty foods that lead to obesity. Certain foods seem to increase or lessen their pain and other symptoms. His weight is more likely than any other specific dietary factor affecting his risk of developing the disease.
