Qualitative Research Critique and Ethical Considerations
Ng, C. A., Fairhall, N., Wallbank, G., Tiedemann, A., Michaleff, Z. A., & Sherrington, C. (2019). Exercise for falls prevention in community-dwelling older adults: trial and participant characteristics, interventions and bias in clinical trials from a systematic review. BMJ open sport & exercise medicine, 5(1), e000663.
The article has focused on the utilization of exercise to facilitate fall prevention among community-dwelling older adults. With regards to the article, there is strong evidence that exercise has played a great role in eliminating falls among community-dwelling people. The article has summarized trials and participant’s characteristics to evaluate exercise intervention among community-dwelling older adults. The study relied on EMBASE, MEDLINE, and CENTRAL as well as other three databases that offered the randomized controlled trials of exercise as an effective mediation to limit falls (Ng et al., 2019). The study reviewed 108 randomized controlled trials. The trials were conducted in 25 countries with most of the trials predominantly having female participants.
The eligibility criteria of the randomized trials included both cluster and individually randomized trials to evaluate the effects of physical activity mediations. The community-dwelling aged adults that were included in the review had to be community dwellers at the age of 60years and above. The characteristics of the trials amid the three different intervention programs that lowered the falls were tai chi interventions, balance and function training as well as programs that had multiple types of exercise. The article has indicated that the characteristics of effective exercise mediations can lead the program providers and clinicians to effectively decrease falls among community-dwelling adults.
The systematic reviews argued that exercise programs can lower the rate of falls as well as the risk of falling among older adults. Deviation of clinical trials and risk of bias in the clinical trials from the study design as well as reporting approaches lowered their credibility. The reporting of trials failed to meet the standards of reporting trials. For instance, most of the trials eliminated sufficient description of randomization and failed to describe the allocation concealment. The review did not include trials that recruited older adults with Parkinson’s disease, dementia, stroke, and hip fracture.
Senderovich, H., & Tsai, P. M. (2020). Do exercises prevent falls among older adults: where are we now? A systematic review. Journal of the American Medical Directors Association.
Senderovich and Tsai (2020) have offered an effective update on multifactorial, single, and multicomponent interventions for fall preventions for community-dwelling adults. The article has revealed that exercise is one of the non-pharmacological fall prevention approaches that have been encouraged for fall prevention among community-dwelling adults. The article has focused on examining whether multifactorial interventions, single interventions, and multiple component interventions including exercise and vitamin D supplementation are effective in reducing the incidence of falls in long-term care facilities (Senderovich and Tsai, 2020).
The article relied on a systematic review design where MEDLINE, Scopus, PubMed, and Cochrane were searched on the basis of inclusion criteria, quality of evidence, and risk of bias. The study included 34 studies after screening different citations and full-text citations. The studies included randomized controlled trials, systematic reviews, and clinical trials. The studies reported that the outcomes of fall risk factors and fall rates reduced significantly due to exercises. Considerably, the article indicated that exercises that are tailored to the risks and capabilities of the participants may be the most effective interventions since they may improve life quality and lower medical costs.
The study focused on the risk of bias to ensure that the study does not offer misleading information. Assessing the risk of bias enables the establishment of transparency of evidence, findings, and results. Considerably, selection bias happens in a systematic review that fails to identify significant information related to the topic. The study assessed the quality of evidence and eligible studies to facilitate the effectiveness of the systematic review.
