This assignment has three parts: 1) Research and summarize an Advance Directive, 2) share your reaction to the Advance Directive, and 3) provide your opinion about Advance Directives. Write three paragraphs, as follows:

Summary (150 words minimum)

Research and locate an example of an Advance Directive form. Find a blank form, not just an article about Advanced Directives, unless there’s a blank form attached to it. Be sure to review the Research Help materials. Summarize the Advance Directive, and identify whether it is Substantive or Proxy. In the Works Cited, be sure to include your Advanced Directive, properly formatted including its title, date of publication, and link.

Reaction (150 words minimum)

Share your reaction to the Advance Directive. What was something that surprised you, or that you found interesting? How do you feel about the kind of information the form asks for? Is there something you agree with, or something that should be handled differently?

Opinion (150 words minimum)

Share your opinion of Advanced Directives, in general. Is having an Advanced Directive a good idea, or is there a better alternative? Do you believe Advance Directives are important? Why or why not?

ANSWER

Research on Advance Directive.

An advance directive is a legal document that contains the medical plans an individual makes about how they want to be treated in future in case they are unable to communicate or make sound decisions. An advance directive form contains two parts; The Medical Power of Attorney and The Living Will. The Medical Power of Attorney gives you the right to appoint an agent that can make health care decisions on your behalf. The agent makes decisions according to your will or the patient’s best interest on how they would want to be treated. The advance directive form requires the agent’s personal details like the name and location. The Living will allows a patient to provide details about how they would want to be treated. In this part the patients provides his /her personal details, life support details which entails how they would like to be treated and when the treatment should be withdrawn, certain life sustaining issues and end life wishes. The advance directive form provides also a witness option.

Reaction.

In the living will part of the Directive Advance Form, it gives one options of treatment and one is able to choose what is appropriate for them .This also means one cannot be handled in any way they did not put down in the directive form. The Advance directive form makes it easy for family and even the health care system in dealing with a patient instead of second guessing about it which reduces stress. In the living will, one also gives detail about when the treatment should be withdrawn and this is great because one is able to get the best medical care and when unacceptable quality of life is noticed, treatment is withdrawn. Basically, the advance directive form makes everything easy for both the health system care and the family. Even though the advance directive form provides the medical power of attorney options, they still could be chances of detoriation in case the agent selected is not available even with a second option agent because decision making powers cannot be shared thus the medication will be provided through assumptions of what the patient would have wanted.

Opinion.

Advance Directives is a good idea because it plays a major role in ensuring that the health of an individual is well taken care this is because the needs of a patient are well presented. The advance Directives also improve the quality of life near death since they are less aggressive and families are in a position of preparing and determining the events that follow before and after death of their loved ones.

Advance Directives allow a patient to refuse certain type of treatment that means one cannot be handled in any way that is not in the directive form thus they end up getting the best health care since they are treated at their own best interest option. In conclusion, advance directives help in achieving good health since the patients are given an opportunity to make vital decisions about their future health thus medication in served to them in the right way.
