Capstone Project Change Proposal

Nurses play a great role as a care giver for patients and help in management of physical needs, treatment of heath conditions and prevention of illness. They have to check on patients and monitor the patient, recording all the vital information that aid in treatment and disease management decision making. However, the nursing role has become complex due to nursing shortages. Nurses are experiencing extended work shifts of twelve hours and more. The long shifts have been popular within hospital staff nurses but most healthcare organizations do not understand the impact of extended hours on patient care and well-being. When the long working shifts are combined with unplanned overtime and consecutive shifts, nurses are at a high risk of burnout and fatigue which may impact patient care.

The impact of long working shifts among nurses has been supported by a number of literature evidences. According to Kaliyaperumal, Elango, Alagesan, et al., (2017) nurses who work long hours are at risk for chronic health problems such as heart disease, obesity, and diabetes. Long shiftwork can lead to fatigue, stress, and burnout (Hegney, Rees, Osseiran‐Moisson et al., 2019). The continuous amount of work has amplified emotional anxiety and burnout among nurses, which diminishes the value of services they offer to patients. (Olley et al., 2019). To address these concerns, there is a need to understand the causes of these problems and identify effective interventions (Vidotti, Ribeiro, Galdino et al., 2018).

The change proposal being proposed to solve the problem of long working hours and shifts is the shift scheduling. The shift scheduling interventions are aimed at creating more regular and flexible schedules. Shift scheduling can result in a positive patient outcome since it reduces medical errors, improve nurse performance, and reduce turnover rates.  Shift scheduling focuses on scheduling of the nursing workforce to reduce long working hours that lead to fatigue and medical errors. Online nursing scheduling software will enable the administrators and managers to schedule nurses more efficiently. The organization will communicate the transition to the nurses in a structured and systematic manner to prevent change conflicts. The shift scheduling intervention is expected to improve the patient care, reduce sickness absence, turnover rate, low-fatigue related errors and improve the nursing working environment thus improving the quality of care.
