o Idea (i.e. what do you plan to implement?)
o Justification/rationale (i.e. why is this necessary?)
o Success metrics (i.e. how will you quantify success?)
o Resources required (i.e. what kind of funding or support do you need?)
o Implementation plan (how will you implement the project?)

Introduction
Loretto Hospital was founded in 1939, championed by a group of community doctors who had a great motive to serve the community and develop a patient-centered hospital that would be at the community’s disposal. Studies show that Lorreto Hospital focused on Community Health Needs Assessment (CHNA) process to pinpoint and highlight health concerns for the community. The hospital would offer treatment services regardless of the patient’s financial status. Research shows that Austin is one of Chicago’s most chronically undermined populations; statistics show that 117,527 residents are African American and more than 75% live below the poverty line. Records from the Chicago Department of Public health show that some of the significant health factors affecting the town include smoking, obesity, minimal access to healthcare services, poor education, and lack of exercise. Research shows that residents do not focus on preventive measures, making them vulnerable to several health issues. Loretto Hospital emphasizes sensitizing the population on the importance of living a preventative lifestyle. The hospital has created the Addiction Centre, which offers inpatient and outpatient mental illness and substance disorder services.
Organizational Description
Loretto Hospital serves the immediate communities that surround it; communities of West Garfield Park and North Lawndale. The Austin community stands as the most significant community getting the services with an approximated population of 97,611 as per the 2016 community survey, with an estimated 24.1% of the population living below the poverty line. The general demographic of the community served by the hospital is 17,499 West Garfield Park residents and 35,423 residents of North Lawndale (Henderson et al., 2020). African Americans dominate the population, followed by Hispanic/Latino and Caucasian sequentially. The socio-economic of the population is median and led by Austin ($31,920), West Garfield ($22,698), and North Lawndale (24,003). The household poverty is high in West Garfield (43.6%), followed by North Lawndale (43.1%) and least in Austin (28.6%). Austin is the most educated, followed by North Lawndale, with West Garfield coming last.
Market Assessment
The hospital carried the community survey and came up with a list of areas with potential opportunities that require service provision. The highlighted regions include access to healthcare services, cancer, heart disease/stroke, HIV/AIDS, infant health, immunization, infectious diseases, substance abuse, tobacco use, sexually transmitted diseases, nutrition, physical activity, respiratory diseases, mental health, injury/violence, cancer, chronic kidney disease, and diabetes. The hospital created the Community Health Needs Assessment primarily to improve the resident’s health status, thus increasing the lifespan, minimizing the health disparities, and maximizing the accessibility to preventive healthcare services.
Areas of Focus for Community Benefits
Some of the areas of interest to the community are preventive care, access to the addiction center, education distribution opportunities, and access to mental healthcare programs. Potential gaps that need to be considered in future healthcare planning the mental health, access to affordable care, a continuation of care, mental-suicide deaths, and immunization (Wallerstein et al., 2017). Some of the potential organizations that the hospital can collaborate with to enhance continuity in healthcare services include the American Society of Addiction; collaboration with schools to follow up on mental health and social stigma, partnership with other hospitals like Amita to enhance wellness and mental health promotion and finally partnering with JSI (a public health body dealing with research and consultation on health systems).
Development of one Equity-Focused Intervention
The primary idea is to implement a plan to educate the intended population on the prevalence, prevention, signs, and accessible treatment for substance disorders. The entire process is necessary since health concerns like substance abuse top the list of problems and must be addressed before the situation worsens; opioids and education are other key community challenges that must be addressed (Bhatt & Bathija, 2018). The success evaluation can be done by taking the admission statistics, substance abuse-related deaths, and general mortality rate. Some of the required resources to see the project through include funding, grants, advertisement and awareness creation, and updates on the hospital website page. The implementation plan involves holding a community fair to educate on the significance of mental health, preventive care, and substance abuse. The residents are encouraged to discuss the cultural stigma; the hospital will also target the youth on social media platforms and sensitize them on the various healthcare concerns in the community that require attention.
