Practicum Experience Plan

Part 2: Individualized Practicum Learning Objectives

Objective 1: To improve my physical examination skills within the next six months.

Planned Activities: When assessing the patients during clinical procedures I will have a comprehensive nursing assessment through the use of palpation, observation, auscultation, and percussion (Kopelman and Dacre, 2019). I will use the necessary tools and applying the necessary physical assessment skills from nursing education in the clinical setting routinely during a physical assessment to effectively determine the general status of the patient (El Sol et al., 2019). By week 24 I hope to be able to conduct a physical examination which includes motor function assessment, cranial nerve assessment, assessment of reflexes, and sensory function assessment through learning from nurses and nurse educators in the clinical setting.

Mode of Assessment: I will rely on meditrek to evaluate, track and manage my physical examination skill. Verification will be documented in Meditrek.

PRAC Course Outcome(s) Addressed: Some of the skills I learned throughout my practicum experience include a clinical breast examination, prostate examination, nail examination, and mini-mental status examination. I hope to utilize these skills to confirm the general status of the patient. However, I want to improve on patient inspection during the examination. This entails effective inspection of areas of the body for normal shape, color, and consistency. I will turn to the nurse educator in case I need assistance when completing these skills and achieving my goals.

 

Objective 2: To improve my critical thinking skills in clinical procedures within six months.

 

Planned Activities: Throughout the practicum period I will focus on actively investigating and scrutinizing a situation or problem to consider outcomes of my options. I will also pay attention to the teamwork approach in problem-solving. I will rely on the Socratic Method at the end of my shift to review the patient progress, history and discuss with colleagues concerning pieces of information that demand special interpretation (Papathanasiou et al., 2014). By the end of six months, I hope to be more independent, making valid conclusions and concluding justifications for patient conditions.

PRAC Course Outcome(s) Addressed:

Throughout the practicum experience, I have been able to carry out various interpretations such as CXR and PTF interpretation. I have relied on critical thinking skills to dig deep into patient’s deterioration, complications, and pains and conveyed meaningful information without misunderstanding or mistakes. I have shown ability in synthesizing and presenting data and diagnostic reasoning. I will refer to my nurse educator for assistance on medically relevant issues that require complex interpretation of data to achieve my goal and gain this skill.

Objective 3: To show great confidence in technical skills such as assessment, diagnosis, and controlling of patient’s health care problems within six months.

Planned Activities: During the identification and treatment of patient problems I will greatly on assessment of patient’s information, collect, validate and document their psychological, physiological, and emotional state. I will focus on clinical and labs that will offer hands-on technical skills. The clinical judgments will enable me in making an educated and complete judgment about the patient (Tollefson, Tambree, Jelly, and Watson, 2018). I am hoping to observe how my nurse educator conducts their assessment, diagnosis, and manages the health care issues. By week 24, I hope to independently conduct an assessment, diagnosis, and management of a patient’s health problem without supervision. I am hoping to enhance my clinical knowledge and skills that influence decisions concerning patient management.

PRAC Course Outcome(s) Addressed:

Some of the skills I have obtained over time include the ability to conduct a biopsy, removal of foreign body, injection of local anesthetics, and diagnostic testing order. To achieve my goals I have to learn things like providing wound care, taking vital signs, administering medication, inserting catheters, as well as starting IVs. I will refer to my nurse educator in case of any assistance regarding skills and competencies necessary for the achievement of my goals and the development of my technical skills.
