Community-acquired pneumonia is a potentially severe illness that is linked with mortality and morbidity (Arias-Fernández, Gil-Prieto, and Gil-de-Migue, 2020). I agree with the first post that a combination of therapy has offered a superior and efficient treatment plan over monotherapy for hospitalized patients with community-acquired pneumonia. Treatment entails mandatory antibiotic therapy and a combination of antibiotic therapy to acquire better outcomes as compared to the monotherapy (Caballero and Rello, 2011). Therefore, rather than only using beta-lactam, Macrolides will be combined with beta-lactams. Macrolides are considered to offer the best coverage for community-acquired pneumonia. For an effective treatment plan, there is a need to have several options depending on the patient’s improvements. Therefore, in case the patient is unable to tolerate the macrolide combination, fluoroquinolones will be used for a more effective combination. Combination therapy will offer better coverage of atypical microorganisms as well as act on tow varying sites on the bacteria.

The second post points out that the patient should remain ceftriaxone 1 g IV a day (day 3) and azithromycin 500 mg IV A day (day 3). I agree with the second post since the clinical status of the patient has improved with reduced oxygen requirements. Therefore, this means that the therapy is working since he has shown the clinical responses that were expected within the three days of therapy. Gastrointestinal symptoms are common side effects. I concur with the post that anti-emetic will effectively eliminate these symptoms and allow the rebuilding of strength. The patient will also have to take Zofran 8mg to prevent nausea and gastrointestinal problems. Considerably, more targeted antibiotics are useful to reduce antibiotic resistance (Nayar et al., 2019). In case the clinical status has not improved, ceftriaxone will be replaced by amoxicillin 1,000mg. The patient will be mobilized within the first days of admission to ensure that the organisms are moved out as rapidly as possible.
