  1. Physical Appearance/Hygiene:
    1. General appearance: Does not have signs of distress
    2. Gender: Male
    3. Apparent age: 38-40
    4. Ethnic group: Caucasian white
    5. Appears overall healthy: The patient appears overall healthy, does not look dehydrated, and his skin is smooth, standard color, with no paleness. Well-groomed. His clothing is appropriate for the weather.

 

  1. Body Structure and Movement (Observe general stature):

Height appropriate for apparent age. 5’10, standard height for his age, BMI is within the obesity range. He appears well nourished, has enough color on his skin, and has no paleness; his weight is inappropriate for his age and height. The weight suitable for his height is 159 pounds and below, but he is 190pounds. They are comfortable and relaxed. He is seated upright and looking attentive. There are no obvious deformities apart from a mole on his neck. He walks with ease, with no visible limp or difficulty in walking. He struggles a bit with breathing due to the weight. Gait is smooth and can maintain balance without assistance. They do not have assistive devices with them. Moves all extremities equally bilaterally and with good posture. There is no visible involuntary movement, and they are not using any assistive devices.

 

  1. Emotional/Mental Status and Behavior:

The level of alertness is high. They are paying attention and can easily answer the questions asked. They are oriented to person, time, and place. They appear well-rested. No signs of eye bugs on their eyes, and they are alert. Their speech is coherent and understandable. They can follow instructions correctly and give back feedback. Behavior is appropriate for the situation. The patient is sitting upright calmly. They do not seem alarmed and acting is appropriately in the situation by nodding and responding to the questions. They are calm, cooperative, and respond appropriately to questions. The patient has also dressed appropriately for the weather. It is hot, and they are dressed in light clothing reflecting the hot weather.

 

  1. Describe two actual or potential risk factors for the client based on the assessment findings with description or reason for selection of them.

The Patient, in this case, is obese. This means that if he does not lose weight and adopt a healthier lifestyle, he is at the risk of becoming diabetic and suffering from heart disease. He is already suffering from obesity, a lifestyle disease, and thus, might need medical interventions to help him lose weight and prevent him from getting other diseases.
