Subjective

The patient is a 56-year-old Caucasian female; she came to the hospital for a 6-month follow-up visit. According to the hospital records, the patient has a history of Hypertension which she has had for 35 years, she has type 2 diabetes, and she is a smoker, as she confesses she smokes 30 packs of cigarettes a year. She is approximately 5’7 and 170 pounds. Her BMI is 26.6, which shows that she is overweight. She has been using insulin to manage the diabetes. She says that she has been experiencing heart palpitations and shortness of breath in the past month. She says that in the past week, she has also had chest pains and pressure on the chest; however, she did not think much about it as she thought it was because she had started a new workout routine.

Objective

The patient is pale. Being Caucasian white, she is supposed to be pink but she is more white than pink. The patient also has a new irregular heart rhythm that was not there in the past and is not in her hospital records. The patient has also gained weight from the last time she was in the hospital, and after asking her, she says that it was a rapid weight gain, and she does not know why she added the weight.

Potential Risk Factors

The patient is faced with several actual and potential risk factors. As a person living with hypertension and type 2 diabetes, she is likely to have cardiovascular complications if she does not stop smoking and if she does not adopt a healthier lifestyle (Maddatu et al., 2017). Smoking is a risk factor for mortality and coronary heart disease in hypertension and diabetes patients. The patent also faces the potential risk of suffering from stroke if she does not stop smoking.
