Subjective

The patient is a 61 year old African male who comes to the clinic complaining of the right leg pain and swelling in her legs. He reports of taking prenatal vitamin and ibuprofen to help in easing pain and swelling. He denies any cramping, pain or skin changes on his legs and denies swelling arms. He is a smoker and takes alcohol occasionally. The patient has a family history of diabetes and hypertension.

Objective

Vitals- T 100.9, P 80, R 18, BP 178/74

On physical examination, the patient is generally cooperative and in no acute distress. Right arm blood pressure is 178/74 mmHg, heart rate is 68 bpm and respirations are 18 breaths per minute. Body Mass Index (BMI) is 35 kg/m2. On examination of the right leg, there is 3+ pitting edema to the mid-calf. There is mild erythema over the medial aspect of the ankle with mild tenderness to palpation along the medial malleolus. There are no skin changes noted on examination of her legs or arms. The skin on both legs is warm to touch with good turgor and tan in color with no rashes or lesions seen.

Actual or potential risk factors

Based on the information gathered from the subjective and objective findings, it is apparent that the patient has peripheral vascular illness. Peripheral vascular system illness entails contraction of the blood vessels that supply blood to limbs (Barrios et al., 2022). The common clinical manifestation is pain in the legs or arms when walking or at rest. The patient’s age, gender, family history, smoking history, and obesity are all risk factors for peripheral vascular disease. The patient has a number of risk factors which have contributed to the development of his condition. These include: Obesity: BMI of 35 kg/m2 puts the patient in the obese range, which is linked to an increased risk of peripheral vascular disease; Hypertension: High blood pressure can affect the blood vessels and lead to peripheral vascular disease; The patient’s age and smoking status are also risk factors for developing peripheral vascular disease.
