Refer back to the interview and evaluation you conducted in the Topic 2 Family Health Assessment assignment. Identify the social determinates of health (SDOH) contributing to the family’s health status. In a 750-1,000 word paper, create a plan of action to incorporate health promotion strategies for this family. Include the following:
Describe the SDOH that affect the family health status. What is the impact of these SDOH on the family? Discuss why these factors are prevalent for this family.
Based on the information gathered through the family health assessment, recommend age-appropriate screenings for each family member. Provide support and rationale for your suggestions.
Choose a health model to assist in creating a plan of action. Describe the model selected. Discuss the reasons why this health model is the best choice for this family. Provide rationale for your reasoning.
Using the model, outline the steps for a family-centered health promotion. Include strategies for communication.

Family Assessment Part II
Introduction
After conducting a family health assessment on the Gonzales family, adequate information regarding their health status, ability to control, resolve, prevent diseases, strengths, and areas of improvement were identified. This information lays a foundation to understand through the SDOH, the complex circumstances that overlap to potentially influence individual and family health or unhealthy practices and outcomes. These elements range from political to social, economic, environmental, education, and personal behavior determinants (Kaakinen, Coehlo, Steele, & Robinson, 2018). This paper discusses social determinates of health (SDOH) contributing to the Gonzales family’s health status, recommends three age-appropriate screening procedures, and uses the Social-Ecological model to create an action plan that promotes the family’s health.
SDOH Affecting Family and Family Status
The Social determinants of health that influence the Gonzales family’s health are education (literacy levels), socio-economic status, environmental determinants, housing, and occupational status. Their general health is good and recommendable, despite having minor health problems and their father undertaking weight management therapy and managing Type II diabetes. The Gonzales family exhibits high literacy levels, with all children having access to quality education even to university/college levels. Having higher literacy levels and access to education shows that they understand better ways of managing healthy lifestyles and maximize medical care access. Through this, the family understands the need to adopt diet and physical exercise regimes to support healthy living. The social-economic status of the family is stable and middle class, living in a well-protected neighborhood. This shows that their surroundings’ status and environmental determinants explain equitable access and distribution to adequate resources and money. This has resulted in the family’s ability to access therapy treatment for the father, the ability to afford eyewear, and the resources to maintain a healthy lifestyle after their father’s diagnoses. The family’s stable economic and financial aspect also explains a possible sedentary lifestyle the father had that resulted in the diagnosis and the technological struggles exhibited. Housing facilities are well established to provide comfort; however, the family’s occupational status and technology are accountable for increased restlessness, stress, and inadequate sleep.
Age-Appropriate Screening Recommendations
Screening for SDOH helps identify the special and core social needs of individuals or families, such as housing, food, transportation, utilities, and persona safety. Procedures integrated into screening ought to be age-appropriate, as the SDOHs that influence the parents’ lives may vary from those of the children. Regarding the Gonzales family, three screening tools, namely SDOH Short-form Screening, SDOH Long-form Screening, and SDOH Patient Action Plan, can be used (AAFP, 2019). For children, specific screening questions that apply include the extent to which they felt secure in their homes, food security, level of childcare provided, and education level they have, focusing on the school facilities and resources. Parents are evaluated based on financial status, occupation, ability to provide basic needs and utilities. Short and long-term SDOH screening procedures help set the health outcomes and goals for the family, and the action plan identifies specific practices required to remain healthy, taking into consideration all the core needs and SDOH.
Assessment of Health Model
The identified health model applicable in the Gonzales family is Bronfenbrenner’s socio-ecological model. This model establishes the effects associated with multifaceted interactions of individual and environmental factors that shape their behavior. The information helps in identifying behavioral and societal leveraging features that help in developing health promotion action plans. The figure below illustrates the model showing that the Gonzales family can create effective health action plans (Kilanowsk, 2017). This model is the best choice for the family. It considers a wide range of dynamics of environmental and individual factors that interrelate influencing personal behavior and provides a combination of health interventions that can be adopted at all levels. Besides, quality health encompasses assessing various elements of their lives that contribute to wellbeing and health outcomes, including conditions they are born, grow, live, work, and age. This model provides a platform to create a comprehensive and conclusive action plan in all levels of interactions, hence considering holistic development and wellbeing in health promotion for the family.
Application of Health Model
Family-centered health promotion in the Gonzales family revolves around maintaining active physical activity regimes, primarily to support the father in his weight management goal, stress reduction, and alternatives to increased rest and decreased technological struggles. Health communication strategies for the family include using verbal, nonverbal, and visual strategies that are culturally competent, especially due to their Latino descent, considering their health literacy, and integrating digital and traditional tools.
