Nursing Problem and PICOT Question

Discuss the link between the PICOT question, the research articles, and the nursing practice problem you identified.
Nursing Practice Problem and PICOT Question
1. What is your identified nursing practice problem?

My identified nursing practice problem is fall-related injuries in elderly patients. Elderly patients higher fall risks compared to other regular patients. It is so because they lose their balance and need a lot of support to stay stable as one ages. Falls have been increasing among hospitalized elderly adults. It has become an issue of concern in both developing and developed countries. Falls among the elderly may hasten adverse medical, physical, social, psychological; and economic consequences

  1. List your PICOT question following one of the template formats reviewed in the course. Summary of studies. Include problem, significance to nursing, purpose, objective, and research question.

PICOT Question: Among elderly adults, how will exercise intervention and programs prevent fall-associated injuries in twelve months?

It is essential for nurses as the patient’s caregiver to offer the best care to every patient that walks into a health care organization. However, falls have made it impossible for nurses to provide such care to elderly patients. It is so because, with falls, the patients are likely to develop new injuries making them overstay in hospitals for further treatments. This project aims to find out the various programs and interventions that nurses and hospitals can put in place to prevent falls in elderly patients. Doing so will make nurse’s work more accessible as they will not spend most of their time on one patient. According to the studies, health care organizations can implement and use different interventions to prevent falls in these patients.

How do these four articles support the nurse practice issue you chose?

  1. Discuss how these articles will be used to answer your PICOT question.

The articles will be essential in answering my PICOT questions in various ways. It is so because they are EBP research work done by other authors regarding a similar problem. For instance, Lee & Kim (2017) focuses on the different ways an organization utilizes exercise to lower falls in elderly patients. The two researchers found that exercise programs, if solely focused on fall prevention, are often effective. Similarly, according to Ng et al. (2019), their research focused more on how exercises are an intervention for fall prevention. Therefore, I will use these articles to answer my PICOT question and conclude the best interventions and programs to help in fall prevention.

  1. Describe how the articles compare the interventions and comparison groups to those identified in your PICOT question.

My PICOT question focuses on the various ways exercise interventions and programs prevent fall-associated injuries in patients. The articles used are one way, or the other focused on interventions used and tested to see their effectiveness on fall-related injuries prevention. The articles and my PICOT question are similar as they have a similar objective; to find out the various exercise programs and interventions that may prevent fall-related illnesses in patients.

Method of Study:

  1. State the ways of the articles you are comparing and describe how they are different.

All the research works used other methods to collect and analyze their data. The authors used qualitative and quantitative methods. They were different because quantitative methods evaluated and compared previous researches, while qualitative mainly derived the data from observations, meanings, and interpretations.

  1. Consider the methods you identified in your chosen articles and state one benefit and one limitation of each method.

The main benefit of using the qualitative method is that it incorporates the human experience. It does this by allowing them to participate in the observations before concluding. The main disadvantage is that it is naturally hard to generate results as, most of the time, the researcher’s view affects the findings. The quantitative method is advantageous because it is more accurate and objective. However, it is disadvantageous because the data may not be clear enough to explain the complex issues to the researcher.

Results of the Study

  1. Summarize the key findings of each of the studies into a comprehensive summary.

The research works had key findings from which I can choose the suitable interventions for fall prevention. First, according to research done by Ng et al. (2019), they found out that most if not all the trials showed that exercises involving functional and balance exercises are essential programs that can significantly reduce falls. Lee and Kim (2017) found out that fall prevention exercise programs lower the rate of falls among elderly patients in the facilities. Senderovich & Tsai (2020) found out an equivalent fall reduction in patients if an organization uses single and multifactorial interventions. Lastly, Zhang et al. (2019) concluded that implementing exercise programs helps reduce injuries related to falls.

  1. What are the implications of the four studies you chose in nursing practice?

Even though the studies are essential in my research and will help complete and answer my PICOT questions, they had various implications. The first research implied that an organization should examine the program’s cost-effectiveness and determine whether it is applicable in the care environment. NG et al. (2019) suggested that the study should ensure it considers both middle and low-income countries.

Ethical Considerations

  1. Discuss two ethical considerations in conducting research.

When conducting research, a researcher is likely to experience ethical dilemmas in the field. The first ethical consideration is prioritizing the participant’s privacy. A researcher should always adhere to the research data confidentiality. Secondly, informed consent is another ethical consideration a researcher should remember. In this case, the participant should be willing to take part in the research, and in case they wish to leave, the researcher should let them go.

  1. Describe how the researchers in the four articles you choose took these ethical considerations into account while performing their research.

The researchers in the articles took ethical considerations by ensuring the participant’s confidentiality and anonymity. For instance, there are no names of patients whatsoever or the participating doctors and physicians going through the four pieces. No one going through the research can know who said what and when.

Outcomes Comparison

  1. What are the anticipated outcomes for your PICOT question?

For my PICOT question, the expected result is to find out how exercise programs and interventions are likely to prevent falls in elderly hospitalized patients.

  1. How do the outcomes of the four articles you chose compare to your anticipated results?

Each of the articles chosen had different outcomes that are relatable to my PICOT question. First, Senderovich & Tsai (2020) concluded that using multifactorial mediations and multiple and single interventions worked well and reduced falls in the patients. Lee & Kim (2017) found out that exercise programs that focus on fall prevention strategies effectively lower falls among adults in care facilities. Similarly to the two, Ng et al. (2019) found out that effective exercise interventions involving functional and balance exercises are suitable for reducing falls among the community-dwelling elderly individuals. Zhang et al. (2019) found out that exercise programs reduce injuries from falling among older adults. Therefore, al the four articles compare to my outcome as they all target using different exercise interventions to reduce patients’ falls.

Proposed Evidence-Based Practice Change

  1. What is the link between the PICOT question, the research articles, and the nursing practice problem you identified?

All the three are linked as they all focus on fall prevention. My identified nursing practice problem is hospital falls in elderly patients. My PICOT question tries to see whether exercise interventions and programs can help prevent the problem. The chosen articles try to answer a similar question. They look into ways various programs and interventions prevent falls in patients.

  1. Based on this information, propose an evidence-based practice change for your identified setting.

For my program, I propose the use and application of exercise interventions that involve both balance and functional exercises. I would suggest this because elderly individuals are losing their balance due to age. Therefore, including balance exercise programs may help them have better credit than they currently do.
