Review the opening scenario of Nam Nguyen in the front of this book. Imagine you are the clinic nurse at the Family Medicine Center. Based on the information presented in the scenario, work through the following questions:

Since he had not had a physical examination for the last ten years, Mr. Nguyen came to the clinic to be established as a patient at the center. Mr. Nguyen and his wife are concern about the knee pain, which Mr. Nguyen has been complaining about have affected his work performance. They find it normal for Mr. Nguyen to have such knee pains since he works as a construction supervisor and has to climb up and down the ladders while checking on things.

Considerably, during a medical examination, it is important to gather effective information about the patient. An understanding of the past and any health data related to the patient or the family background can be effective in offering insights on what is happening presently (Rosdahl and Kowalski, 2008). The nursing process begins with a health assessment done to offer a comprehensive assessment of the mental, physical, socioeconomic, spiritual, and cultural status of Mr. Nguyen.

The Nursing assessment pays attention to examining the physical responses and the client’s functional abilities. Physical examination is considered as the technique of gathering data about the body as well as obtaining data about the body system. For the case of Mr. Nguyen, I would act as an advanced nurse practitioner with the ability to assess, examine, treat, make diagnoses for the patient. Considerably, as an advanced nurse practitioner, I could be able to make decisions that are based on the diagnosis and assessment as well as the interpretation of the test results.

Nursing Process

The nursing progression works through an efficient guide that offers patient-centered care through five progressive phases. These steps include assessment, diagnosis, planning, implementation, and, finally, evaluation (Treas and Wilkinson, 2013). Assessment is considered as the initial step that includes a physical examination, assessing the patient’s health status, and collecting objective and collective data. When asking Mr. Nguyen about the reasons for his visit, Mr. Nguyen is at the initial phase of the nursing progression. The diagnosis phase is the second stage in the nursing process. The diagnosis stage involves activities such as analyzing the patient’s data, identifying the health risks or problems, and the formulation of the diagnostic statements.

With regards to the assessment and diagnosis, the nurses can set achievable and measurable short terms as well as long terms goals for the patient, such as maintaining sufficient nutrition and management of pain through sufficient medication. The goals are well stated for the health professional can construct a plan that will direct personalized care for the patients. The evaluation phase is ineffective in the case of Mr. Nguyen since the interventions have not been implemented, and interventions can be reassessed. Considerably, evaluation and reassessment vary according to the health status of the patient.
