INSTRUCTIONS:
You will add the analysis/diagnosis section to the PDF from the last module and upload it into Canvas.
Using your text construct a nursing diagnosis for all four of the characters/clients.

Assessment: What do you notice about the characters? What color are the characters? How would you describe the character’s joints? Voice? Hair? Mobility? What does the character state?
Diagnosis: Using your nursing diagnosis book, write a nursing diagnosis using the correct format in the text. Pick an approved nursing diagnosis from the text that most closely fits the assessment data of the character. Do your best, the purpose is to familiarize yourself with analyzing your assessment data.
Planning: Once you have assessed and analyzed/diagnosed, you are now ready to formulate goals for the client. Write two short term goals for each character. The goals should be clear, concise, and measurable. What do you think will benefit the client? What is the client’s goal? Is the goal realistic?
Implementation: This is the doing/action phase of the process. What nursing interventions are needed to accomplish the goals you created in the planning phase? You can use your text for ideas.
Evaluation: How will you know that you accomplished your goals? Remember the nursing process is cyclical so this phase involves re-assessment. Maybe your goal was unrealistic? Maybe your goal was accomplished? What kept you from reaching your goal? Maybe you need more time to evaluate?
